There are two ways a claim is filed with the insurance provider. For services that your provider billed directly to your insurance company, you do not need to file a claim as long as proof of insurance was provided at the time of service. If you paid the medical expenses or services out of pocket and would like to file a claim, you will need to provide medical records, bills, and a claim form to your plan administrator.
